Instructor Verification Form

Use this form to verify that an individual has obtained a YMAA teaching certificate and at what level. The check will also tell whether they are currently covered by YMAA insurance.

Type the Members First and Last names into the boxes provided, be sure to use the correct spelling and enter no other character (such as commas or periods), select confirmed in the confirmation menu and click the send button for on screen verification.
